In an effort to decrease the usage of wasteful and error-prone in-clinic paperwork completion, Solv is thrilled to offer a digital registration solution to help decrease wait times and data entry errors by allowing patients to complete their paperwork digitally before they arrive to the clinic, or on their mobile device while they are in the waiting room.
Edit the paperwork fields
Review the fields included in our standard paperwork registration by going to Settings > Locations > Edit > Paperwork to indicate the which fields you would like to be Optional vs Required. You can also enable or disable the fields you would like patients to see when completing their paperwork.
Be sure to click save after any edits you make!
Solv provides the option to add up to 5 custom questions to the standard paperwork flow to gather any additional information required for patient registration.
Additionally, Solv provides the ability to add custom consent forms (i.e. consent to treat, notice of privacy practices, financial agreement, etc) for patients to review and type their name to electronically sign.
Please keep in mind that these consent forms will not support the ability for patients to enter information (i.e. name, date, or checkboxes). To have consent forms uploaded, please ensure that they do not contain any lines for information to complete, as patients will be able to review the form content, and then type in their name one time to agree to all forms.
Edit the patient's paperwork
To edit any fields in the patient paperwork after it's been completed, go to the Queue, click on the patient name, navigate to the Paperwork tab and enter free text or select a value from the dropdown menu and click save.
You have the option here of copying the field value (ex: patient's name) and pasting into the EHR, or downloading the completed paperwork as a PDF using the black circle download button and then uploading the entire paperwork PDF into the patient's chart in your EHR.
Steps to download the patient's paperwork
In the Solv queue, click on the patient's name and to the right of the Info tab, you'll see a Paperwork tab and a Consent tab where the patient's forms can be downloaded and uploaded into the EMR.

Please note that to maintain privacy and security, paperwork and registration forms will disappear after 72 hours of the patient's visit, so please download forms from Solv and upload the information into the EMR as soon as possible.
