Custom registration questions
Solv provides the option to add up to 5 custom questions to the standard paperwork flow to gather any additional information required for patient registration.
To add custom questions to your paperwork process, please email the support team ([email protected]) or log in to manage.solvhealth.com and select the “Live Chat’ option at the bottom right of your screen.
When sending an email, please have the subject line state ‘Request to Add custom Paperwork Questions”. You can also state the same when opening a chat.
When requesting to create custom paperwork questions, please include the name of your custom questions (ex: Are the patient's immunizations up to date?), where you would like the questions presented (registration, patient’s face sheet or via your kiosk), field type (input, select or checkbox) and which locations you would like your new custom questions displayed.
If requesting via chat, the support team will also ask these questions to set up your custom questions.
Once the customization request is complete, confirm with the support representative that the changes have been made according to your specifications.
Custom consent forms
Solv provides the ability to add custom consent forms (i.e. consent to treat, notice of privacy practices, financial agreement, etc) for patients to review and type their name to electronically sign.
Please keep in mind that these consent forms will not support the ability for patients to complete information, like name, date, or checkboxes. To have consent forms uploaded, please ensure that they do not contain any lines or fields for information to be input, as patients will be able to review the form content, and then type in their name once to agree to all forms.
To have consent forms uploaded, please reach out to the Support Team via email at [email protected] or via chat (login to manage.solvhealth.com and select ‘Live Chat’ at the bottom right hand of your screen).
Please ensure that all consent forms are in PDF format and do not contain any lines or fields for information to be inputted, as patients will be able to review the form content, and then type in their name once to agree to all forms at the end of the paperwork process.
Form accessibility expiration
Please note that to provide the highest level of patient data security, paperwork and consent forms will only be available up to after the patient's scheduled appointment. After this time limit, they will not be available for you to download and save.
